What To Bring With You

  • New Patients: To help prepare for your first appointment and to save time, please download our New Patient Welcome Packet. You’ll see that one is a survey so we understand your condition, three documents require your signature and the remaining three are educational about the program. Please read and complete as requested and bring them along to your first appointment.
  • Your Vermont state issued Medical Marijuana Card indicating CVD as your designated dispensary. You will need this at every visit.

Cancellation/Inclement Weather Policy

We understand things come up! We no longer charge a cancellation fee but if you can’t make your appointment, please give us a call so we can make the time available for another patient.

It is our mission to ensure that our patients have access to their medicine. We do our best to remain open in the event of inclement weather so that we can honor scheduled appointments. Please check our homepage, Facebook, or call us for up-to-date information on delays or closings, which generally follow school closings in Chittenden, Addison and Windham Counties due to inclement weather. Our main office hours may vary so if you have any questions regarding your appointment, please call or email us at support@cvdvt.org.
